Employee felt pain in neck and shoulder when they manually closed a rail car door on October 20, 2025. They sought medical attention on January 29, 2026 and was prescribed physical therapy.

EE was working on a CAT 990H attempting to remove the loader arms. In the process of removing the pin between the Zbar and hydraulic cylinder, the Zbar pivoted momentarily pinching EE's left upper thigh between the Zbar and the cylinder.

Employee was opening a metal gate on a chute. The top hinge bolt was missing and as the door opened, it fell forward and pinned EE's finger between the door and the handrail. The employee sustained an injury to EE's right index finger requiring stitches.

EE scraped his right shin while entering cab of drill when his right foot slipped on step. This happened approx.2 months ago but employee didn't seek medical attention until 12/8/11. He was treated by a doctor on 12/8 for an infection. His wound was cleaned and dressed. No restrictions or lost time. Incident is being reported within 10 days of seeking medical attention.

Employee was working outside, disassembling an H4000 crusher. While carrying 5 gallon buckets of oil, he became overheated. He was taken to the doctor and was diagnosed as having heat exhaustion.

EE was about to help co-workers change v-belts on crusher when he began to not feel well. EE had taken his blood pressure that morning,. EE said he did not feel good, light headed and somewhat dizzy. Co-workers told EE to sit down in the shade and rest. He was diagnosed with heat exhaustion by doctor.

AT 7:30 AN EMPLOYEE COMPLAINED OF SEVERE CHEST PAINS VIA THE CB RADIO. CO-WORKER FOUND EE ABOUT ONE MINUTE LATER SLUMPED OVER CONTROLS OF LOADER. SUPERVISOR AND CO-WORKER DETERMINED EE WAS NOT BREATHING AND HAD NO PULSE, PERFORMED CPR UNTIL RELIEVED BY EMT'S. EMPLOYEE TAKEN TO HOSPITAL; PRONOUNCED DEAD @ 8:45 AM.
