Unchoking a chute with limestone. Jammed right shoulder.

Tripped over ultra vac hose and used left arm to brace themselves. Felt a pop in shoulder.

While using a high pressure Jetstream system, the water/air did not completely dead head upon releasing the foot pedal controlling flow, the wand in use became dislodged from port of preheater and stream struck employee on the back of the left leg (knee and calf area).

Turned around on a catwalk and lost footing. As falling hit right torso and right cheek on top of handrail. Diagnosis contusion to both areas. Was given restrictions from appt same day. F/U appt on 08/01/2025 received "Return to Work Full Duty".

Using a large wrench and it slipped off a nut, momentum caused person to fall on back. After diagnostic visit (07/10) placed on restrictions.

Employee was walking and tripped. Fell on left shoulder and head on concrete surface. Diagnosis through emergency room visit: cervical sprain; contusion left shoulder, minor head injury (concussion). Employee will be off work at least two (2) days. No witnesses, undetermined what employee actually tripped on. Area in question as been reviewed and no obvious obstructions.

Walking and slipped in grinding aide liquid, hurt left ankle, 3 broken bones, immediate surgery

Employee hung a torch on a railing above the employee's head. The torch fell and hit employee on the right temple causing a small cut by the right eyebrow. Two (2) stitches were needed. No restrictions were enforced. Employee returned to work after medical treatment was performed.

Using a vac hose, the suction pulled EE forward strain/pulled EE's lower back. Initially declined medical attention. Pain increased and requested medical on 02/12/2025, which were able to schedule. Restrictions were placed with that appointment.

Hammering on a block, the hammer skipped on the edge and ended up hitting the employee's right hand/wrist. Employee did not seek medical attention until 01/20/2025. With this appointment employee was placed on restrictions of 10lbs push/pull, 10lbs carry/lift with right wrist. Employee came back to work after appointment with brace on wrist.

Closing a clay belly door on airslide when it went positive and blew cement dust out and went into eyes. Employee was wearing safety glasses it was just an unexpected plume of material.

Working on clearing a choke. Bin went into positive pressure. Material flushed. Employee turned face and material entered side through glasses. Debris in right eye. Eye Clinic ER. Went off rotation per schedule. 1st Regular work day was 12/17/2024. F/U appt on 12/18/24. Received clear report.

Stepped wrong walking up stairs and landed hard on left foot. Seeked medical attention on 12/12/2024. Sprained left foot. Prescribed a boot and was put on Sedentary Work.

Employee was fixing a light fixture in the AF ER Room. Had the wrong breaker turned off (labels are on breaker panel (1) Building Lights (1) ER Room). Used tick tester and it was faulty. Showed no voltage. Employee grabbed connector and received electrical burn.

Employee was using an air lance to clean out a diverter gate. Gate threw and knocked air lance pinning left hand/wrist against housing.

Employee was called in early for a equipment failure. Slipped on loose pile of clinker. Felt a strain in middle back.

Employee stepped off a ladder and twisted right knee. Diagnostic treatment began 10/22/2024. Surgery performed on 11/18/2024.

Individual getting out of skid steer, right pant leg got caught, used left leg to brace themselves and it cause left ankle to roll.

Walking down stairs, swinging arms and hit right wrist on support post of handrail.

Took a few steps backwards in the mechanic shop and fell over some material (which was not in a walkway). Thought was further away. Fell on right side and used right arm to brace self. Injured right wrist. Placed on restrictions after Dr. visit 05/06/2024.

Walking down stairs, foot caught on stairs, left knee buckled under. Possible meniscus issue. 1st Dr. appt 05/07/2024. Will have MRI scheduled. Placed on restrictions as of 05/07/2024.

Stepped off forklift and slipped. Used right arm to keep them from falling. Diagnostic through Ortho (X-ray). Possible rotator cuff issues. Put on restrictions from Dr. visit 04/19/2024.

Walking on rocks and tripped. Fell on right arm to try and catch self. Jammed shoulder. Diagnostic treatment and Therapy. Surgery performed on April 15, 2024.

Cold conditions employee was in layered clothing had to climb several flights of stairs

Walking on a catwalk and stepped on a rock. Strained right knee.

Incident is from repetitive motion from a pre-existing post surgery performed 2021. DOA is day employee decided the need to seek medical treatment. Diagnostic tests which determined Arthroscopy, should with bicep tenodesis has been ongoing. Surgery performed on 07/16/2024. No lost time nor restrictions up to this point.

The employee slid the left pointer finger under the element to feel for an obstruction when the element slipped into place falling approximately 25mm. As the element fell, the finger became logged, causing a smash injury to the finger. On 10/6/2023, the employee was taken to the Dr for evaluation, where the employee was given antibiotics.

Cleaning out preheater, was near an open port and material "burped". Employee had backed away and opened visor on Helmet which allowed material in right eye.

Changing out a cooler on a compressor, picked up cooler and strained back. Diagnostic treatment. Surgery 10/16/2023. Bone chips removed from vertebrae.

Employee was carrying a piece of pipe on shoulder and ran into a door jamb. This action caused a jarring in employee's shoulder. Diagnostic treatment, surgery was required which occurred on September 18, 2023.

Drill steel came out of the carriage and fell in to the housing. We then laid the tower down to maneuver drill steel back in to place. While doing this the drill steel kicked back knocking EE off the drill rig on to the ground. EE sustained a nose bleed and back pain. We then called an ambulance and EE was transported to a hospital.

Walking down a set of stairs which were slippery from rain. Slipped and fell. Jarred left shoulder. Diagnostic treatment. Surgery 10/24/2023.

Cleaning out an air slide when it "burped" and debris got into EE's right eye.

Walking on uneven surface and hyperextended left knee. Diagnostic treatment until surgery was performed on 06/26/2023 for a torn meniscus.

Employee was walking through an area and material flushed and employee got material in left eye.

Walking on uneven ground and sprained right knee. Diagnostic treatment. Problem reoccurred. Surgery performed to clean out cartilage on Aug. 30th.

Employee was replacing a lid for a chamber in the main dust collector penthouse. Lid was being replaced using overhead crane. Lid needed approximately 2" adjustment to slid into place and when adjustment was made the lid quickly popped into place and employee had finger between lid and frame. Finger was smashed in between lid and frame causing injury to right hand middle finger.

Employee strained right shoulder while operating a come-a-long in an awkward position. Diagnostic treatment until rotator cuff surgery performed on June 22, 2023.

While making a blind lift, with 3 flagmen, the tip of a mobile crane bumped an overhead beam, dislodging buildup, that fell and contacted the left hand of the repairman rigging the lift. Employee suffered fracture of left hand. Employee did not require surgery, and is in a cast, on restricted duty.

EE was sliding/adjusting forks on forklift, when fork pinched EE's fingertip.

While installing brick in a kiln a shim driver was used to help install shims for the new install. The shim driver jumped off of the shim, hitting the employees left, pinky finger.

Working under some floor grating. EE reached up to hold an I-beam and reposition self when some dust/dirt from overhead was dislodged and fell into left eye.

Using an End Grinder while wearing safety glasses and face shield, a gust of wind come up causing some debris to kick up and get into EE's eye.

Was tightening bolts when one of the wrenches EE was using slipped off the bolt. The bolts were at eye level and the employee was standing while conducting this work. EE was using an adjustable spud wrench and a box end wrench to tighten bolts. Due to the force and angle, the spud wrench slipped and struck them under right eye.

While rebricking the kiln, a pogo slipped off the brick it was holding causing the brick to fall on the employee's finger.

Employee was turning away to leave from the Raw Mill Sampler, when EE tripped over a wooden block. Employee did not immediately seek medical treatment, then subsequently underwent surgery for torn tendon.

Employee was working concrete with 4' Bull float and several extension handles. Employee was repositioning when employee tripped on the gravel subgrade & fell on right knee on the rough terrain.

While performing Gunite operations the hose plugged. When the hose was cleared it released causing the nozzle to kick back striking an employee in the side of the face.

Bent over cutting a 1/4" steel plate with a Metabo grinder. When EE stood up EE had a catch in their back.

Employee was cleaning the windshield of the crane with a vinegar/water mixture. 15 minutes later they started to feel a burn in eyes.

EE was hitting a stiffener ring on a duct with a 16 pound sledge hammer. The hammer glanced off the ring pinching EE's finger between the ring and the hammer handle. EE was wearing gloves.

Fitting pipe at Kiln pier 3 and stepped backwards and stepped off the concrete onto the ground on a rock causing EE to sprain right knee.

Trimming off black expansion material with utility knife and using the knife in an improper manner cutting towards self. Cut through the glove causing a laceration on EE's left hand.

Lifting a piece of pipe (8 inches round, 5 foot long) up to the top deck on the 7th floor of the preheat tower to be installed. Thinks they strained right elbow lifting pipe.

Forklift operator dismounted forklift, moved an empty plastic tote, and strained left shoulder. Medication was prescribed at primary medical facility, awaiting MRI for further evaluation.

Was putting a conveyor chain in and it got stuck. When EE got it unstuck it pulled EE's left hand into the conveyor cutting the tip of EE's index finger.

EE was operating a chain hoist while looking up when dust fell from the hoist. Some of the falling dust got around EE's safety glasses and entered EE's right eye causing irritation.

While ascending up a scaffold ladder, EE stepped back on an unlevel surface of the deck and lost balance causing them to fall to the deck below which resulted in stitches to eye brow and minor back injury.

Working inside the cooler installing bars. Went to transfer a bar to its correct position and lower back popped.

EE was removing an extension cord from the cyclone. Dust blew into right eye around safety glasses. EE washed the eye out with eye wash solution but said that the eye was still feeling like something was in it.

Employee was welding when employee felt something in right eye. Had the proper PPE on.

Employee was cleaning up on cooler pad and sudden excruciating pain in left elbow. They took EE to be checked out and they said it was gout. This is the reason I did not file a 7000-1 because it wasn't work related at the time, then they wanted another opinion and they took them to see a different doctor August 3 and they said EE has an inflamed bursa and probably some tendonitis.

Employee was tightening bolts on expansion joint at the chiller pad. Ratchet slipped and struck employee on EE's right cheek.

Miner was grinding steel duct work. Wind blew grinding dust up under face-shied and safety glasses into miners right eye.

Employee was putting scrap metal in dumpster. A piece of metal fell over the dumpster to the other side and the employee was moving it back onto the forks when it sled off onto EE's left foot.

Employee was in a scissor lift and was cutting a piece off of the cable tray with a cordless porta band. Employee was holding the material with EE's left hand when it slipped and cut two fingers on left hand.

Employee tripped on buildup along catwalk, injuring right knee. Physician diagnosed later as torn meniscus, and surgery was performed 12/1/21 to repair.

Employee tripped and fell over a pipe and bruised shoulder and sprained back trying to catch their fall.

Miner was using a 6" grinder. Grinding wheel struck left index finger. Miner went to clinic, was examined, and given a tetanus shot. Was referred to emergency room for further treatment. After examination and negative X-Rays the physician cleaned the cut, applied ointment, bandaged, and applied a metal finger guard. Anti-biotic pills were prescribed.

Employee was manually lifting mast of light tower via crank mechanism. Safety latch on crank mechanism broke while employee was attempting to adjust grip. Upon the latch breaking crank handle spun at a high speed and struck employee's left hand as employee attempted to bring it under control.

Employee stepped up on kiln pier, and felt knee pop. Physician prescribed brace and walking/climbing limitations.

When climbing ladder to loader, foot slipped on step. EE used left arm to break fall, by holding handrail. Employee continued working until surgery (11/20/20) to treat shoulder damage.

Employee was moving conveyor pans, when they felt a strain in lower back. Reported the following day, continued to work until physician prescribed injection. Restricted to no step climbing at this time.

Employee was swinging a sledgehammer, when finger was caught between the handle and the manhead pin, resulting in broken finger. Physician prescribed restricted duty.

Employee was troubleshooting instrumentation on kiln, as they broke the union loose, an airblaster cycled blowing their glasses off, and depositing dust in their eyes. Physician cleaned out eyes, and prescribed medication.

Employee was climbing down a ladder, when EE stepped on a air hose fitting, and rolled EE's ankle. Dr prescribed a support boot and restricted walking.

While repairing the limestone weighbelt, employee pulled/lifted parts, causing a stinging sensation in upper back. Employee worked without restrictions until physician diagnosed as bulging discs on 6/6/19, and prescribed limitations and epidural block.

Employee had been operating dozer, using ripper on uneven and hard material. Over the course of 2 days, employee began to have discomfort in neck. Physician prescribed no restrictions, until surgery was performed 10/16/18. Expected a few lost workdays before returning on restricted duty.

Employee stumbled off of concrete pad, braced self with left hand, causing discomfort. Physician initially did not find any problem. After discomfort persisted, employee was sent to hand specialist, whom found a small bone chip, which was subsequently removed surgically on 10/30/18. Employee has returned to work, with restrictions.

EE missed step descending stairs at truck load out, as EE stumbled EE held onto handrail, straining the right shoulder. EE reported injury, but did not request medical attention for 2 weeks, until discomfort continued. Physician prescribed anti-inflammatory medication, and lifting restrictions.

Employee was walking down steps, then slipped on landing between steps. Employee did not fall, but strained right knee. Employee reported the next day, physician prescribed limited climbing.

Employee was feeding grinding balls into feed end of ball mill with another employee. Other employee dropped a 3" grinding ball on ankle, as well as slipping on some of the other balls that had collected on the ground. Physician treated inflammation with stationary boot and restrictions.

Employee was swinging sledgehammer to dislodge manhead for Ball Mill, sledgehammer bounced off of manhead. As Employee sought to control the hammer and keep it from falling, they strained a shoulder. Employee did not feel medical attention was necessary until a week later, as discomfort continued. Physician prescribed lifting restrictions and physical therapy.

Employee was jack hammering buildup in cement elevator, when ee felt strain in lower abdomen. EE did not initially report, but as discomfort persisted, reported on 8/4/17. Physician diagnosed as thrombosed hemorrhoid. Procedure to drain resulted in restricted duty.

EE was using heavy pry bar to clean material from clay shredder, when strain occurred. EE did not feel immediately necessary to be evaluated medically. approximately 1 week later, physician diagnosed as thrombosed external hemorrhoid. Procedure performed to drain, employee prescribed restricted duty.

EE strained while moving a partial drum of mill balls, physician diagnosed as thrombosed external hemorrhoid. Medical treatment to lance and drain, and restricted duties.

EE tripped, caught self with right arm, injuring shoulder. Employee reported incident, but did not feel that EE needed medical attention until 2 weeks later. Doctor prescribed medicine and issued lifting restrictions.

Pressure was applied to roller mill hydraulic system to check for leaks. A leak was detected at fitting, which was tightened to stop leak. The fitting broke loose while tightening, and struck EE in the ribs. Physician diagnosed as contusion, prescribed limited lifting etc.

Employee lifted service truck brake drum to install on rear axle, when EE strained back. Employee reported, but continued to work for approximately 2 weeks, until EE requested to see physician. Physician diagnosed as pinched nerve.

EE TURNED OFF BREAKER TO THE S BLOWER, DID NOT PLAN TO WORK ON BLOWER, SO DID NOT LOCK OUT. THEN WENT TO CHECK INFO ON N BLOWER, EE REMOVED GUARD, AND LEANED OVER TO GATHER THE TAG INFO ON THE MOTOR. RIGHT HAND CONTACTED BELTS, AND THE BLOWER STARTED AS THE SPOUT BEGAN LOADING A TRUCK. FINGERS WERE PULLED BETWEEN BELTS & PULLEY, 2 FINGERS WERE CUT.

EE was un-choking air slide with air lance, as material began to flow, the material backed up and blew out. EE moved back, then stood and struck forehead on beam. Injury required minor stitches.

Two employees were inside quarry crusher fines chute cleaning built up material. A large piece of buildup struck the walkboard causing the employees to fall onto the material. One employee chipped bone in elbow as it struck the bin. The other employee was not injured.

Employee was chipping slag with a jitter bug. a piece of slag shot up and burned the area between the bridge of the nose and the left eye.

Employee raised up from crouched position and struck nose on part of beam structure. Physician sealed laceration with superglue and prescribed antibiotic.

EE was driving shim between refractory bricks, when ee struck thumb with the hammer. Physician treated thumbnail and prescribed antibiotic/anti-inflammatory medication.

As EE reached for handle of machine shop door, a co-worker pushed the door open from the other side, striking the left hand of EE.

As EE was pulling vac truck hoses into position, he slipped on ground wet from rain. Physician prescribed limited duty.

EE was carrying a portable air tugger that slipped out of his grasp, as he caught the falling tool, his right elbow was strained. EE continued regular duties until dr prescribed surgical repair of partial muscle tear. Employee will work with restriction of right arm until released.

EE was removing blower, when he squatted down & felt right knee pop. EE continued working for following days until dr. prescribed no climbing restriction.

EE was replacing the seat on a pay loader, as he was removing the old seat, he turned and felt a pop in his right knee. He continued working until surgery was performed to repair torn meniscus.

EE was dragging vac-truck hose, when the hose caught on something, and strained elbow. Physician prescribed restricted duty until MRI determines damage.

EE was standing inside discharge chute when foot slipped causing knee pain, dr prescribe no climbing restriction until MRI determination of injury.

EE was located beside dock ramp, when another employee pulled pin releasing dock to loading position on truck. EE reflexively extended arm as dock sprung up. The hinged end of dock plate extended up and pinched fingertip of employee. Tip of finger was removed. EE returned to work with restrictions after one lost workday.

EE pulled on tail drum of primary crusher undercrusher belt, assisting placement when he felt pain in lower back. EE continued to work, and reported incident 3 days later. Physician prescribed limited duties on 10/3/13.

Employee was vacuuming rock spillage, when jackhammer bit fell from platform above, bruising and scraping his side. Physician prescribed restricted duty.

Employee was assisting the emptying of dust collector hopper & unchoking tipping valve when he reached inside of the tipping valve to remove a piece of material. The screw conveyor below the tipping valve removed middle fingertip. Employee returned to work after 1 day away from work.

employee was going into the chute door when he felt lower back pain. Employee continued to work regular job without issue, until physician prescribed job limitations on 3/22/13.

Injured Employee was Asst. Plant Manager, supervising an employee using an air lance to clean material behind mill rolls. The small wire had been used to secure the fitting became unsecured and the fitting struck the ee in the mouth. Dentist prescribed antibiotic for lower lip injury.

EE reported (3 days later) back discomfort, and was unsure of activity that caused discomfort. Discomfort did not restrict EE job assignments until 2/4/13.

National Cement was notified by rescue services, that an individual jumped off of highwall of non-active flooded quarry. The individual did not resurface and recovery actions were initiated. Immediately called MSHA hotline, and explained that the individual was trespassing on property, and was not a work related fatality. MSHA inspector arrived and conducted investigation.

EE, along with other workers, were lifting small set of crossover steps, when he felt pain in his back. Physician prescribed modified duty.

EE felt pain in abdomen while at work, but when reported, was not sure of activity that caused discomfort. Physician diagnosed as hernia, and EE underwent surgery on 8/16/12.

EE had cleaned the area underneath the crusher, EE was returning the wheelbarrow when he fell against the wheelbarrow, bruising his ribs.

EE was disposing of scrap roller assembly with another employee. Later, EE felt a bulge in his abdomen that was tender. Physician diagnosed as hernia, surgery was performed on 8/16/12.

Employee was holding a pin as his partner was striking pin with hammer. The hammer hit the side of the pin, and struck the employee's hand. Physician prescribed anti-inflammatory medication. Employee was not limited in work duties.

As ee was striking the outside of a chute to clear material inside, the rubberized hammer he was using skidded/slipped off chute, causing employee to strain back. The incident was not reported, the employee continued to work, on 10/18 the employee called in without explanation, and did not work that day. On 10/19, after reporting the incident, physician prescribed limited duty.

Employee was removing seal from kiln trunion housing, when tool being used slipped out of the handle, straining right bicep muscle. Physician prescribed limited use of arm.

Employee slipped while descending stairs from lab. Steps were wet, but had anti-slip material on steps. Employee had been experiencing pain from non-work related plantar fasciitis previous to incident, but stated that the slip caused an increase in pain. Physician prescribed limited use of foot.

Employee descended haul truck ladder, did not fully engage foot into bottom rung, allowing the foot to slip off of rung and employee fell approximately 22 inches to ground. Physician prescribed limited use of knee.

Employee was cutting steel on band saw and dropped a piece that was on the roller bed onto his foot. Physician diagnosed as contusion, prescribed short term limited work restriction.

Employee was walking on inclined catwalk, when he felt something pop in his upper leg. Physician diagnosed as torn hamstring. There was no slip or trip involved.

EE was using jackhammer, lost balance and fell forward catching his foot in the floor grating. Physician prescribed protective boot and limited duty.

EE slipped on step to control room that was wet from rain. Physician prescribed limited use of knee.

Mobile Equipment Supervisor dropped bucket tooth on right hand while examinining tooth. Requested medical evaluation following week (9/13/10). Physician prescribed anti-inflammatory medication with no restrictions.

Employee stated that he stepped on hard material while walking and twisted his knee.

EE was in process of moving bags of cement onto pallet when he strained his back. Physician prescribed limited work duties.

EE reported on 6/9/10 that he strained his back on the previous day. EE stated that did not know, definitively, what had caused the discomfort. Physican prescribed anti-inflammatory medication and rest.

EE slipped on loose material, resulting in contusion to elbow. Physician prescribed limited use of arm.

EE strained shoulder dislodging buildup with bar. EE requested medical examination approx. 1 1/2 month after incident due to continued discomfort. Physician prescribed anti-inflammatory medication but no restrictions. **EE underwent surgery for continued discomfort on 11/11/10. One lost workday and 60 restricted workdays resulted from this surgery.

EE was installing dust collector bags as his finger was struck by bag cage that another employee installed. Physican prescribed medication for wound.

EE experienced discomfort in lower back while operating loader, but did not feel that medical evaluation was necessary, until approx. 4 months later when he asked to see a physican due to recurrence of discomfort. Doctor prescribed modified duty.

EE reported that while moving calibration plates, he strained his hand. He reported months later that he had problems with his hand since incident, though he had continued normal work activities. Physician prescribed restricted use of hand.

Shift Supervisor opened port of dust collector airslide to check for blockage, when material surged, blowing dust in face.

Employee attempted to set kiln shell fan, which was bumped, pinning hands between fan and rail.

EE stepped down from steps onto approx 3"x6" patch of ice located on platform grating, slipped, and caught finger in floor grating as he fell.

EE stepped in gap in floor between kiln and hood platform while adjusting forklift mast.

EE rolled ankle while dismounting dozer. Physician prescribed limited use of ankle.

EE attempting to move shaft with pipe wrench, which slipped causing EE to twist ankle and injure nose on handrail.

EE was welding inside crusher. After returning to work the following day, EE reported that his eyes were "burned" from reflected glare of welding. EE was wearing welding hood. Subsequently, EE realized there was a small metal shaving in eye. Physician removed shaving.

EE struck bearing with hammer causing a metal splinter to be expelled, striking EE and became embedded in forehead. Physician removed splinter.

EE was supervising the clearing of a choked chute, when he stepped onto an air lance and turned left foot. Physician diagnosed as slight fracture of bone in foot.

EE was using a sledge hammer to tighten bolts on raw mill drive motor, and reportedly missed target, straining muscle in his back.

EE jumped off of back of maintenance pickup truck and turned right ankle. Initial doctor examination reported no significant finding. Later examination showed slight fracture. EE was prescribed to wear walking boot brace, but no restrictions, but due to no safety toe on boot, management placed EE on office work duty.

EE was using water lance to clean material in conditioning tower when large piece fell and struck lance, causing employee to lose balance and felt pain in lower back. Physician prescribed anti-inflammatory medication.

EE was walking outside, slipped in mud caused by rain, and caught himself on right hand.

EE attempted to pull rubber strip that covered rail car scales, when he felt a strain in his abdomen. Physican diagnosed as hernia, surgery was required to correct.

EE attempted to dislodge rock from hitch area of truck, straining his back.

EE was cutting rubber hose with pocket knife, when knife slipped off hose and cut right hand. Physician applied 3 stitches.

EE reported back pain during extended shifts incurred during maintenance outage. Physician reported degenerative discs, and prescribed work restriction. EE underwent surgery in 2006 for back injury.

EE was pulling tarp on clinker pile, when he felt strain in abdomen. EE had undergone non-work related hernia surgery approx 1 year prior, of which he received full release to work. Upon current incident he went to physician as precautionary measure. Physician prescribed restricted duty as precaution.

EE was operating hammer drill periodically through shift on Friday, after being off for weekend, EE reported shoulder pain Monday. Physican prescribed anti-inflammatory medication and work restriction.

EE was cleaning ground level concrete pad under primary crusher when he slipped and caught himself with his right arm to keep from falling. EE reportedly strained his right elbow, and on 11/25/08 asked to seek medical evaluation. Physician referred EE to physical therapy to ease discomfort.

EE hurriedly dismounted John Deere loader to check status of clay feeder. As he dismounted, EE neglected to insert his entire foot into the bottom rung of the ladder, but only caught the rung with the toe of his boot. As his weight shifted to dismount the loader the toe slipped from the rung and EE fell to the ground, catching himself on his left hand, injuring his left wrist.

EE was burning a bearing off of shaft, hot grease from bearing contacted his left cheek and ear, causing minor burn.

Employee was removing jib from crane. As jib swung around employee caught finger between jib and crane boom.

EE slipped on one step cross over twisting his foot/ankle. Physician discovered hairline fracture in foot.

EE took medical leave to have disc surgery. While awaiting surgery, employee filed for work comp. and received doctor statement of work related cause of medical condition. There has been no specific accident or date related to this condition.

Wind blown dust blew into eyes.

EE tried to secure door on preheater vessel, receiving burns to arms.

Two separate incidences: 02/02/07 Struck brim of hat on low hanging pipe. 07/01/07 Struck head on ladder. Had block on 8/14/07 for pain, lost day reported 8/17/07 Surgery to correct neck pain on 01/08/08.

EE received radiant UV from arc welder, causing eye discomfort.

EE was climbing a ladder when one foot slipped off of rung causing him to fall approximately 4'.

EE was exiting the kiln and struck burner pipe with face.

EE was moving portable conveyor with other employees. Other employees lifted before EE causing conveyor to fall on his left foot.

The EE was walking out of the kiln when he struck the burner pipe. He sustained contusions to the back of his head and left shoulder.

Employee was climbing onto a loader with a handrail, when he sustained an unknown injury to his right elbow.

The employee was pulling a bindicator out to measure the truck silos. When the employee checked the silo dust/material blew out of the bindicator pipe and onto his face/eyes.

EE was walking, careful not to slip on wet ground(rain), and struck brim of hardhat on low pipe. Incident resulted in strained neck, but did not require medical attention at that time. EE's neck discomfort increased and bumped head on ladder on 07/01/07. EE had myelogram procedure on 08/14/07, which resulted in lost workday. EE has long history of neck/back pain.

EE was lifting a pallet (approx 40 lb.) from roller bed. Due to past back problems, EE may have overcompensated during lift and strained his shoulder. EE did not lose any work days after accident. EE has experienced discomfort in shoulder and has opted to have surgery on shoulder. Lost workdays have begun on first lost day due to that surgery.

The employee was pulling a pallet off of the top of a stack of pallets and the pallet under it fell off, hitting his ankle.

Employee picked up a sledgehammer and knocked out a metal shim under a steel plate. Allegedly a metal shaving ricocheted off & struck employee in the abdomen embedding the fragment of metal.

The employee was mounting a bearing on the shaft of the raw mill separator bearing housing. While walking with the bearing, which is heavy, from the work bench to the unit, he stumbled. The weight of the bearing put a strain on his lower back.

The employee was asked to demonstrate, in a classroom, how to properly disengage a safety disconnect. He used his left hand, looked away, and pulled the switch hard. When he pulled, his hand was cut at the base of the handle frame.

The employee was working on a step ladder. He over-extended himself & fell. The EE was originally injured on 4/17/06, but this did not become reportable until 5/09/06.

At the end of the day the EE went to the shipping office to put on his jacket. When he put it on he felt a sting from a spider.

The EE had just reported for work at 5:00am. As he was walking down to the lab basement he slipped on the first step and fell against the wall, injuring his right rib cage.

The employee was installing a split rubber seal on a dust collector door under silo 50. He was cutting the seal material with hisutility knife when it hung up in a corner of the door. He reaced down with his left hand to free it and it came free cutting his left index finger.

The employee was moving an overhead 5-ton hoist by pulling the chain. He felt a strain in his right shoulder. This actually occurred on 10/5/05 but did not become a reportable injury until the employee had surgery on 5/15/06.

THE EMPLOYEE WAS ATTEMPTING TO MOVE AN OLD ULTRA-VAC HOSE OUT OF THE WAY OF THE FORKLIFT. THE EMPLOYEE WAS WALKING ON LOOSE CLINKER AND TWISTED HER LEFT FOOT.

THE EMPLOYEE WAS WORKING ON AN AIR CYLINDER. WHILE REMOVING THE PIN ON THE ROD WITH THE AIR TURNED OFF, THE ROD CAME OUT, STRIKING THE EMPLOYEE IN THE MOUTH.

The employee was trying to turn a screw with the belts on the reducer to align the bolt holes, so as to replace a bolt. While performing this action the belt pinched his right middle finger.

The employee sustained a strain when a large rock was dropped from a payloader into the bed of his dump truck. This took place on 6/20/03 but the injury did not become reportable until 5/24/05 when the employee missed work due to the doctor taking him off work.

Employee was driving a rock truck down a hill when the left front tire of the truck came into contact with a berm causing the truck to flip over.

While moving a drum of grease with the drum trucks, the drum shifted back hard on the handles. When this happened the ee felt something pop in his left shoulder.

While the employee was attempting to clear a choke in the preheater, hot material flushed out of a door burning his legs.

The ee was attempting to clean buildup in a chute with a punch bar. the punch bar slipped resulting in the ee right hand being mashed into the side of the chute. as a result his right fingers were injured.

Two employees were working on a reducer. It would not move so as one employee went to jog the belt, the other for some reason had his hand on the belt. The belt started and cut the end of his fingers.

The employee was attempting to climb onto the excavator machine when he slipped. His left shoulder impacted the track on the vehicle.

Employee was lifting a bag of sand from the floor in the lab, when he felt a sharp pain in his lower back. He sustained a strain.

The employee was leaning over while welding on a conveyor screw. When he rose, he pulled something in his back. (not reportable until 10/12/04)

The employee was attempting to remove an old chain link from a drag. The employee and his partner were using comalongs when the hook came off and hit the employee's thumb.

The employee was climbing a ladder to change lightbulbs in the Roller Press building. While doing this he pulled a muscle in his lower back.

The employee was walking to the storeroom after lunch. As he was crossing the railroad tracks his foot slipped and went into a hole in the top of the switch cover. he fell and twisted his right ankle.
