Employee slipped in some mud while fueling employee's drill and twisted left knee. Employee sought medical attention on June 27, 2023 and was diagnosed with a sprain and released to full duty. Employee returned to the doctor on July 7, 2023 for a follow up and was prescribed physical therapy, continuing full duty.

Removing an oil cooler from a 775F. Employee had to attach a come-along to it to assist with lowering it out of the machine. The cooler got hung on the machine. Employee freed up the cooler, causing chain to slide to one side of the cooler. The cooler then mashed employee's finger between it and a guard.

Employee exited the company vehicle to talk to supervisor, who had pulled up beside EE in their vehicle. Employee's right leg was pinched between EE's door and the other truck door. Injury became reportable on March 12, 2019 when employee was prescribed an ankle/foot orthosis.

Employee was assisting in the removal of a counter-shaft from a 7 foot cone crusher. During the process, the counter-shaft shifted causing the employee to fall against a metal plate. Employee received a laceration to upper thigh and is currently on restricted work duty.

Employee was operating a torch to cut a wear plate for the load out bins when something blew over his PPE into his right eye. He was diagnosed by a physician with a corneal abrasion. He was treated with Alacaine drops and discharged to full duty.

EE received 1st & 2nd degree burns to L forearm & hand from an electric arc flash. Appears EE left testing wire in place on main breaker & energized the circuit; noticed during post job inspection. Did not request immediate medical attention until he was driving home (30 - 45 min. later) and the burn continuously worsened.

Employee sustained a concussion when he struck his hard hat against a welded safety strap underneath the bed of a haul truck. It occurred when he stepped up on a 30 inch ladder to grease the rear of a haul truck. The blow rendered him unconscious for a short period until his co worker woke him up. He start to go to sleep a second time but was kept awake by his co worker.

Employee had been welding metal skirting on the jaw feeder. After completing the welding the employee stood on the bumper block and pulled his leads up. When he attempted to step off the block, he slipped and fell two feet to the concrete pad.

Employee was removing guard off of drive shaft to work on transmission and felt pain in his groin. He did not request medical attention until 02/02/06 and was diagnosed with an inguinal hernia. Physician placed employee on restricted duty and refererred to a specialist/general surgeon. The specialist will do surgery at end of February.

Driller was cleaning drill area for preparation of drilling at the Huntsville Quarry. While using front end loader a 25' - 30' section of wall fell onto the cab of a Cat 980C loader. Employee received scratches to face and 4 sutures to left pinky finger. His left pinky finger was broken.

Employee crushed his left index finger; he was changing out a old punch plate from #1 screen. The chain came loose causing the punch plate to come down and crush the employee's finger.

EE HAD A SLIVER OF METAL TO ENTER THE PALM OF HIS RIGHT HAND WHILE HE WAS EXAMINING CLEANING THREADS ON A 1 1/4" BOLT. THE METAL WAS SURGICALLY REMOVED FROM A LIGAMENT IN THE EE'S HAND.

EE WAS DRIVING OLD BOLTS OUT OF CRUSHER WITH A BAR AND SLEDGE HAMMER. EE STRUCK THE BAR AND CAUSED A PIECE TO FLY OFF AND CUT HIM ON THE CHIN.
