EE moved a 50lb welding rod container to a job site location to begin welding. As EE began to set the container down, EE's left wrist came in contact with a sharp bur on the can resulting in a 2 inch laceration that later required 7 sutures to close. The EE was wearing the required glove, but the cut occurred above the cuff to an exposed area of skin.

Miner was cleaning off a catwalk and allegedly felt discomfort in the shoulder area. After an investigation, it did not seem plausible the injury had occurred on the mine site. The miner ceased contact with the company until a State (Alabama) Panel Exam authorized the injury and the employee contacted the employer on August 25, 2022.

EE was cutting a conveyor belt for removal. In the process of cutting the belt EE cut the left elbow. EE received a laceration to the left elbow resulting in 4 stitches.

Around 10:00 AM a co-worker found Employee lying on the ground by the service truck apparently having a seizure. He was unresponsive to questions. He had been sitting in the truck with the air conditioner running since around 9 AM. He was taken to the hospital by ambulance. He passed away at the hospital.

EMPLOYEE WAS WAITING IN HIS TRUCK AT THE PRIMARY CRUSHER. AS PART OF THEIR JOB, THEY ARE TO REMOVE ANY SMALL ROCKS FROM ROADWAY TO PREVENT TIRE DAMAGE. HE HAD A FREE MOMENT AND DECIDED TO MOVE SOME ROCKS. EMPLOYEE HAD REMOVED SEVERAL STONES WHEN THE LAST ONE CAUSED A STRAIN IN HIS RIGHT FOREARM. THERE WAS MINIMAL LIFTING BUT MOSTLY LATERAL MOVEMENT.

EE WAS HELPING POSITION LOADER BUCKET ONTO FLAT BED TRUCK. A CHAIN WAS BEING USED TO POSITION THE BUCKET. THE INJURED EE PLACED HIS RIGHT HAND AROUND THE CHAIN. WHEN THE BUCKET WAS LIFTED THE RIGHT HAND WAS CAUGHT BETWEEN THE CHAIN AND THEBUCKET.
