Operator hired contractor ee for the first time to replace tires on a Case 721 wheel loader. He was to supply all parts, tools, equipment, and labor. Experienced miner escorted contractor to the wheel loader. Contractor did not inform operator of any hazards that might be created by his works. He was underneath loader when the loader fell off two jacks onto him.

2 registradasEmployee was placing wire around a pallet of stone. He disreguarded the backup alarm of a Case 95XT skid steer loader that was moving in reverse. The employee disreguarded his safety training to yield the right-of-way to moving equipment. As a result, he was struck by the loader and died from his injuries.

Employee was working on the discharge side of the stone splitter stacking stone on pallets. Went to pick up a stone that approximately weighs 40 pounds when the stone rolled over and hit employee's right hand which resulted in a fracture of middle finger.
Skid steer operator was getting off of employee's machine when employee stepped down employee's foot landed on a small stone which caused employee to twist employee's ankle.
Truck driver was strapping down employee's load and got too close to the edge of the truck when employee lost employee's balance and fell.

Employee was splitting a large block using a hammer to drive wedges into shims. Block separated and shifted striking his foot. He ignored training and procedures by not using S.L.A.M. method and not properly bracing/shimming block prior to splitting.
