Employee was prying some hardened material when pry bar slipped and caught EE's thumb on a handrail.

Employee slipped and fell on wet track pad injuring possibly shoulder.

Employee was pulling a lifting hook away from grinding tire when hook struck a finger which required 3 stitches.

EE clearing plugged pipe when material came down on poke bar forcing EE thumb into pinch point. Requiring stitches. Immediately returned to work.

Employee came to work and sat down to put boots on and pulled lower back when they bent over needing medical attention.

Employee was welding in a restricted area that required you to light the torch prior to reaching work area due to lack of room. As the employee was accessing the work area the touch hose got hung up on a thermo coupler on the floor causing the hot torch to go across EE's hand.

Employee was walking from EE's vehicle to the time clock to punch in to begin EE's shift when EE slipped and hurt EE's shoulder.

An employee was prying a chunk of material with a bar which slipped off the chunk, causing employee to lose balance and then went backwards into structure behind EE. Employee did not miss any work or no restrictions until the shoulder flared up again. It was determined EE has a slight labrum tear requiring surgery. Employee became a reportable and had surgery on 12/16/2020.

Employee was walking up stairs when employee missed a step and began to fall. Employee then braced self and fell on right hand breaking finger.

Employee was lifting a #100 bag and felt discomfort in EE's shoulder/arm. EE was sent to physician for examination. Slight bicep tear. Returned to work with restrictions. This was not submitted due to error.

Employee was cleaning build up from the preheat tower when a nickel size piece of hot material fell into EE's boot and burned EE's ankle. Ankle was treated by medical professional but then became infected and unable to return to work on 1/24/2020.

Employee was exiting silo floor when EE turned to leave EE felt something pop in EE's left knee. No restrictions until Nov. 29th.

Employee clearing cement blockage got dust in eye.

Employee dropped a pneumatic jackhammer on finger tip and cut the end of finger. 2 stitches required.

Employee was rotating a paddle blade upwards to align a shaft and set screw when the edge of the blade caught their glove and pulled it into a steel flange fracturing their left thumb.

Cement dust in employees eye.

Emp was cutting a piece of steel with Iron Worker. While cutting piece it folded under cutting emp Lt. hand index fingertip between the steel and the framework of machine. Finger was bandaged and no stitches / No lost time/restrictions for five (5) days to keep cut clean.

Employee states they slipped on step getting into the excavator machine twisting left knee. Date of reported injury was 6/22/2018 and employee had surgery 3/20/2019. No lost time in between.

DRIVER WAS BRUSHING CEMENT DUST OFF OF ROOF OF CAB WHEN EE STEEPPED BACK AND WENT TO THE GROUND

Emp was removing kiln feed end steel band when dust fell to the floor causing dust to chimney up under safety glasses. Treatment was flush Rt. eye and prescription eye drops.

The employee was injured while hand loading material onto a freight elevator. The employee fell forward breaking Rt. ulna and lacerating head when the elevator car floor shifted and employee lost balance. Employee was injured on 3/28/18 was referred to Orthopedic Specialist and returned to work with restrictions on 4/2/18.

Emp was clearing out a clay hopper with an air lance when EE struck a hand rail with EE's Lt. hand middle finger. Emp was wearing gloves. Emp pinched the tip of EE's finger between lance valve and rail causing a fractured tip. Lt. Hand middle finger fracture / splint.

Employee was poking #10 clinker vibrator in clinker tunnels when dust fell from ceiling area and blew into employee's Lt. eye. Prescription ointment was administer at clinic.

Emp was cleaning riser duct of 4th floor of north PreHeat Tower when draft went positive causing dust to puff out. Dust went under face shield going into Lt. eye Prescription drops were administered.

Summer help were chipping and cleaning handrails on catwalk. Emp walked by area and stopped just as a piece of material bounced off catwalk and struck ee in the upper lip. Laceration / One Stitch

Employee operating the Quarry End loader was working into a pile of shot rock when they were jarred and then felt a pop in their left shoulder. EE was sent to Occupational Health Department and was placed on restrictions 7/19/17 that will not allow them to perform full duty.

EE is a Welder Repairman was changing v-belt on conveyor while installing finger became caught between belt and sheave. EE was sent to Occupation Health for a pinch to tip of little finger Rt. Hand. Over the counter meds were given. EE stated that the tip of finger was bent inward. EE was sent to Orthopedics and a splint was placed on finger 7/25/17. Continues to work Full Duty.

Employee was removing a roof sheet when dust particles came through the roof and went into the right eye.

Employee was using a water blasting gun to clean riser duct of kiln. EE turned head just as a puff of dust came out of duct causing foreign material to get into Lt. eye. Sent to ER and examination showed abrasion of eye / Prescription Eye drops were used. No lost time came back on next scheduled day.

Employee was operating the Haul Truck in Quarry. EE stated the jarring and back and forth motion of truck caused back to hurt. EE was sent to Occupational Health 9/7/2016 & was placed on restrictions. Returned to work on 9/8/2016 on light duty.

THE EMPLOYEE WAS CUTTING WITH A TORCH ON THE HOPPERS WHEN A SPARK FLEW UP UNDER WELDING HOOD AND SAFETY GLASSES HITTING EE IN THE RIGHT EYE. EE FLUSHED EYE OUT AT A WASH STATION & THOUGHT THAT, EVERYTHING WAS OUT OF EYE. ON 04/26/16 EYE CONDITION HAD WORSENED THO.

Contractor MSHA I.D.#S146 All-Around employee was using a cutting torch when something flew in Rt. eye under welding hood and safety glasses. EE did not report injury until 04/26/2016. Object was embedded.

Emp was transferring cement from Old Silo 13 through screw conveyor while air lancing angled pant leg cement flushed came out of door and struck him in the face. Emp was wearing eye protection, but did get dust in his eyes. He was sent to emergency room to flush eyes and was given prescription eye drops.

In removing the cooler grate beam from the cooler. The beam became wedged. When the beam came free it shifted to the left pinching EE Finger between the beam & handrail.

Employee stated on 01/22/2015 employee had discomfort in employee's Rt. hand from performing employee's tasks of Road Truck Driver. Employee continued to work full duty until 12/11/2017 when carpal tunnel surgery was performed. Employee is a lost time as of 12/11/2017.

Emp was changing silo 23 to silo 20 and was hitting selector in an upward position w/slegehammer w/Rt.hand. Emp struck Lt.hand index finger. X-rays showed three fractures and a splint was placed on finger. Restrictions of "no Lt. arm work". Emp continues at in his same position training a new PA. No lost time.

Emp was operating Volvo end loader when he noticed a flash as he dug into pile of rock. He continued to operate when he noticed black smoke. He then realized a hydraulic line broke and was smoking. He retrieved the fire extinguisher mounted on handrail of loader and bumped his Rt. elbow on handrail. He was sent to Occ Health on 6/4/14 he received restrictions of no work Rt. arm.

EE was removing a Keeper off of Pin on Spillage Chain and did not realize his Thumb as in the way.

Emp was using a poke bar to clean out vessel on PreHeat Tower. While poking struck Lt. hand 5th finger (pinky) between edge of vessel door frame and bar. X-rays showed fracture of bones at tip of finger and nail was drilled to relieve pressure.

Emp was welding, grinding and burning. At some point a piece of metal entered his Rt. eye. Emp thought he flushed it out, but his eye was bother him when he returned on Monday, 03/17/2014. He reported it and was sent to eye specialist. A piece of metal was imbedded in the cornea of eye.

Emp was sweeping floor of Mobile Equipment Shop when a co-worker saw him stagger and fall to the floor. Co-worker went to assist emp and called for emergency help. Co-worker determined a pulse and emp was breathing with some difficulty. Emergency help arrived and started CPR. Emp was transported to hospital emergency room and at that time ER stated emp was deceased.

Emp tried to pull hood open on semi to put washer fluid in container. Pulling on hood handles, pulled hard on hood and it did not move. Emp felt pain in Lt. Shoulder. Emp continued to work until surgery on 3/19/2014.

Emp was starting work when he saw a door open to grease room. Shut door to grease room and walked up steps. Grease on the bottom of his boot caused him to slip on steps twisting his leg. Employee did not miss any work until MRI showed torn tendon. Emp had surgery 12/23/13 and became a lost time reportable.

While checking beltline with co-worker, co-worker went up to top of hopper to unlodge a rock that was wedged. Hard hat fell off of co-worker onto beltline below. Emp attempted to grab hard hat and ran, slipping, and falling on his Lt. side. Strain to Lt. Shoulder, continued complaints of discomfort, examination showed tear / Lost time on 01/29/14 when surgery was performed.

Emp was attempting to unplug vessel at PreHeat Tower. Emp was at the tipping valve location, valve had become stuck with material. Emp pulled down, valve broke loose causing emp to strike his Rt. hand on metal plate. (10 stitches)

Emp was operating dozer when he ran over a large rock jarring his Lt. arm. Emp continued full duty work until he complained of discomfort and was sent to Occupational Health were he received restrictions. No operating dozer on 3/21/2014.

Going to ladder on top of tanker trailer, drivers foot slipped and he fell to ground. Back and elbow pain.

Emp had #108 belt shutdown and was starting to clean out mud from the drum area under tripper using a poke bar. Bar became lodged between drum and guard emp Lt. hand struck edge of guard causing laceration to thumb. Stitches (wearing gloves)

Miner was lifting 12'x8" channel overhead to be welded in place. Miner reported to the employer on May 23, 2013 that he had pain in the lower back. Employer offered medical treatment but miner refused. on 5/24/13 miner requested medical treatment and was placed on restricted duty. The project was 100% complete on 5/24/13 and the miner was laid off at the end of shift.

On 02/1/13 Emp. was operating a 60 ft manlift when he went into a rut in road causing basket to jar suddenly causing emp to strike mouth with arm. Emp. reported a loose tooth on 02/13/13 sent to Occ Health for exam. Orthodontist removed loose tooth on 02/25/13.

Emp stated on 03/26/2012 he was installing the door (approx. 50lbs.) on the 710 elevator with three other employees. As they lifted the door he felt a pop in his Rt. shoulder. Emp continued to work until having arthroscopic surgery on 01/14/2013.

Emp states that he went to his doctor for discomfort in his Rt. and Lt. hand/wrist. Doctor stated he had bilateral carpal tunnel and it was work related. Emp had surgery on 12/26/2012 and is off work. Emp stated on 3/21/11 that he had discomfort in hand/wrist continued pull duty until surgery date 12/26/2012

Emp was placing door 5" x 14" (10lbs.) back into place at kiln riser duct 5th floor preheat tower. While tightening T-handle screw door slipped and started to drop employee caught door with Rt. hand on bottom of door which then caused the tip of his Rt. hand middle finger to strike edge of door below. X-Ray showed fracture to tip of finger. Emp was wearing heavy gloves

Emp reported he strained Rt. lower back when using a poke bar to unplug rotary feeder for fuel mill. Bar slipped causing emp to twist back. Emp was examined at Ill. Valley Com Hosp - Occ Health on 11/06/2012. It was determined emp could not work due to prescription drugs (muscle relax) he was taking.

Emp. was cleaning out PreHeat Tower vessel with high pressure water blaster gun. Lrg. chunk of matl. fell striking gun causing emp. to fall backwards, pulling gun out of vessel as emp. fell. Trigger struck on gun causing it to go out of control/high pressure water struck emp. in lower leg. Laceration/stitches.

Emp. was removing side liners on #4 Loesche Mill (Raw Mill) seal with 8mm allen wrench. While loosening bolt, wrench slipped causing emp. Rt. hand to strike edge of mill roll. Laceration approx. 3" long required 14 stitches.

Emp was lifting a chain out of toolbox. Emp stated he strained his Rt. shoulder while lifting. Emp continued to work at his position, when discomfort continued an MRI was performed and showed a tear, emp elected to have surgery on 04/28/2014. He is a lost time as of surgical date.

Employee was exiting the cab of the excavator holding onto the handle on side of cab with Lt. hand and opening door with Rt. hand. Stepped out onto tracks of machine, slipped on muddy tracks, let go of door and held on handle with his Lt. hand causing all his weight to shift to his Lt. arm. Emp. remained at work until surgery was scheduled to repair torn rotator cuff on 02/29/2012

On 12/06/2011 employee was pulling wire up to a wire tray in motor control room. While lifting the heavier wire with his Rt. hand and untangling smaller wires with his Lt. hand felt a pain in his Rt. thumb. Emp was being treated with anti-inflammatory medication until examination of Rt. thumb trigger finger was determined. A trigger release was performed on 02/01/2012.

Emp. completed his task using a bobcat while exiting machine stepped out on uneven surface causing his Rt. leg to twist. Emp. continued full duty work while treating Rt. knee pain over the counter medications. Continued discomfort required further examination showed a Rt. knee meniscus tear. Surgery was on 01/09/2012. Lost time began on 1/10/2012 due to emp. shift.

Emp was inside 605 elevator running a comealong on the outside to remove a link out of chain. In the process of pulling the chain the pulling device on the chain straighten out came off of chain cause chain to strike emp in the back of head on hardhat and below hardhat. Emp received a laceration to back of head that required staples.

Emp was working with co-worker reinstalling headpulley cover. Co-worker was lowering cover from platform above pulley to emp below using a chainfall through an opening in platform. Cover got caught in opening as emp was attempting to loosen it - it broke free causing cover to spin and flange struck his upper lip. Returned to dr on 04/13/2011, lip was determined to be infected.

Emp was using a pry bar to remove a bent fender bracket from a semi. Emp cut bolts holding bracket in place and took pry bar to remove bracket. Bracket broke loose causing emp to loose balance, causing his Rt. hand ring finger to strike the battery box, causing a fracture to finger. A splint was placed on finger.

Emp was placing top covers on a screw conveyor. Emp was tightening bolts when wrench slipped off bolt causing Rt. index finger to strike cover causing a laceration that required 4 stitches.

Emp. was holding a box wrench under a conveyor belt clamp, while taking clamp off, bolt broke causing clamp to snap apart. Angle iron on clamp struck Lt. ring finger, causing laceration that required 6 stitches.

On 01/21/2010 emp was installing Finish Mill liners using a com-a-long and guiding liner into position / liner moved suddenly emp reached out to stop movement and felt strain in Lt. shoulder. Emp was examined /possible tear in shoulder emp. remained at job and attended PT. Due to continued discomfort emp had surgery on 05/18/2010

Employee was sitting on the top area of the #5 cooler fan damper housing using a rachet to tighten bolts. Employee's hand slipped off rachet handle causing employee to loose his balance and fall 10ft to the floor area below. Employee was not wearing / using proper PPE.

Two repairmen walked up a flight of stairs to check the #341 dust collector. The 1st man turned around & observed the other man sitting on the steps on the last flight of stairs leading to the platform. While checking the dust collector, he heard a loud boom. He observed co-worker laying face down on the motor control center roof, 33' below the steps where he had been sitting.

Employee was climbing ladder to enter the Randon Water Truck. As employee reached the platform he felt he might have slipped or missed the hand rail causing him to fall backwards off the steps/ladder. He struck the ground causing a contusion to his Rt. hip and Fracture to his Rt. wrist.

Emp was working in #5 Finish Mill removing mill liners. Emp was prying under liner while other emp was using a 20lb. sledge to loosen liner. Pieces of liner broke off striking emp in the upper Rt. arm and upper leg. Sm piece was removed from upper arm stitches were required. The piece in his upper leg required surgery- LT as of 08/28/09.

Employee was welding, cutting, and grinding when his Rt. eye started to feel painful and swollen. Employee stated he thought he might have a welding flash. He was sent to eye specialist for examination and a piece of steel was embedded and removed from his Rt. eye.

Employee was working in the Finish Mill, saw the bobcat operator moving a 55 gal. barrel from under a dust collector stand. He went to help rotate the barrel around to help the operator, as he grabbed the top of the barrel, the t-bar attached to the bobcat pinched his Rt. hand pinky finger against the barrel and caused a laceration that required 5 stitches.

On 5/5/09 emp. was tightning bolts with wrench when it slipped causing emp. to fall forward striking Rt. knee on I-Beam. Emp. remained at his position until 10/28/09 emp. states that he aggrevated his knee while running to assist an emp. during an emergency at the plant. Emp. had surgery 12/18/09 and is a LT.

Emp was working in the battleship (coal-coke bldg), welding & grinding in the area when the wind blew material into his eyes on 04/22/2009. He flushed eyes & felt fine. Eyes became swollen & red over the weekend. Employee noticed a spot on his eye that was not there before & reported the incident on 04/27/2009. Went to eye specialist who removed embedded piece of metal.

Employee was temp transferred to Welder Repairman. He was welding and cutting in coal/coke storage building. Wind blew pieces of dirt and metal into employee's Rt. eye on 04/04/09. He stated it was removed but discomfort continued he reported on 04/06/09. Sent to eye specialist on 04/06/09 - piece of metal was embedded and removed Rt. eye.

On 01/05/09, 5:30am ee was using a poke bar to loosen frozen material in rock hoppers. During his shift he felt discomfort in his Lt. arm, felt it was jarred while poking frozen material - he has been treating with Physical Therapy - further testing has indicated a torn rotator cuff in Lt. shoulder. Employee has become a lost time as of 03/31/2009 pending surgery.

Emp was working on pantleg of clinker area pan conveyor. Torch was ignited in kiln area causing a blast of air/dust to travel down pipe to opening in pantleg where emp was working causing emp to move backwards striking his neck on pipe. This occurred on 10/17/2007 emp has been in PT but continued discomfort has required surgery to Rt. shoulder 08/04/2008

Employee was entering the excavator climbing up the ladder to the machine. He slipped on ladder bumping Rt. leg and holding on rail/handle with Lt. arm/hand to stop his fall on 06/26/2008. Employee remained at work until surgery on 09/12/2008 for a ligament in his Lt. wrist that was repaired. Employee remains off duty.

Employee was working on 8th Floor of PreHeat Tower. The wind blew material into employee's Lt. eye. Employee did not report until 06/17/2008. He was sent to Eye Specialist. Foreign material was embedded in employee's Lt. eye and was removed.

Employee reported discomfort to his Rt. hand on 06/13/2008. Employee continued to work at his job title while attending Physical Therapy. Doctor's examine determined contraction of three fingers of the Rt. hand. Surgery to release/repair contraction of the tendons was performed on 11/07/2008. Employee is a lost time.

Employee was changing a conveyor belt in the clinker storage area. The employee was exposed to clinker dust for an extended period causing irritation to eyes, skin and throat. He had a burn from the dust to the ankle and his eyes and throat were irritated causing discomfort.

Kiln was in a upset condition producing hot clinker. Diverter gate malfunctioned causing hot clinker to remain on the 434 belt. Belt started on fire.

Cement Kiln was in an upset condition, kiln discharged hot clinker which caught 454 belt on fire. Supervisor discovered fire at 7:02 am and called the fire department. Fire was extinguished at 7:41 am. There were no injuries as a result of this accident.

Employee was poking out the clinker vibrator with a poke bar. Employee went to put the poke bar away in a metal locker when bar slipped and pinched employee's Rt. hand pinky finger between bar and metal locker door. Employee was taken to Occ Health for examination and laceration required stitches.

Accident occurred on 11/20/07 employee did not seek medical attention until 11/30/2007. Surgery was 01/28/08. Employee was decending stairs on a belt conveyor line when he slipped while grabbing handrail felt strain in his lower back,Rt. side and groin area. It was determined employee had an umbilical hernia. Employee remained at work at his job until his surgery date.

Employee was splicing #434 belt while securing/pulling the conveyor belt he felt a pain in his Lt. shoulder on 05/29/2007. Employee was sent to the doctor and returned to work with restrictive duty of no heavy use Lt. arm and continued in his maintenance position. Discomfort continued and employee had surgery 09/17/2007 and remains off work.

Employee was entering breakroom closing door behind him. Pulling door with his Lt. hand holding door on edge - caught hand between door and jam. Contusion to the index and middle finger required stitches. Employee remains at work in his job position.

Employee starting the high pressure water gun to clean the PreHeat Tower. Due to the sub-zero weather gun may have had a piece of ice in hose/employee reached to turn valve when the gun came on causing water stream to strike employee's Lt. Ankle cutting his boot top and lacerating his ankle. Employee has been hospitalized to receive intravenous antibiotics to prevent infection.

On 02/02/07 employee was walking from vehicle to clockhouse through parking lot when he stepped on a rock causing his foot/ankle to turn. He reported immediately but did not want treatment. Continued discomfort, employee was sent to Occ Health and an air splint/cast was placed 02/07/07.

Employee was cleaning off feed shelf at feed end of kiln. While closing the door he caught his right hand in between the door and frame causing a contusion to ring finger. He was sent to hospital and finger nail was drilled to release pressure.

Employee was lifting castings out of the kiln on 11/10/2006 when he felt a strain in his Rt. shoulder. Employee was examined and continued to work at his regular job with Lt. duty restrictions on his Rt. arm. Employee was scheduled for Ortho surgery on 12/18/2006 for torn ligaments in his Rt. shoulder. Employee has lost work days as of 12/18/2006 and has not returned to work.

Employee was opening the inspection door on the 568 Rotary Feeder door swung down striking Rt. hand finger between the door and the feeder. Employee was sent for examination and stiches were required to the tip of finger. Employee was not a lost time.

Employee complained of pain in his Rt. and Lt. hand thumbs on 02/24/2006. Employee was examined and doctor determined employee had a repetitive condition of trigger finger with pain relieved by cortisone shot or surgery. Employee decided to have surgery on 12/11/06 and 12/27/06. Employee is off work as of 12/11/06.

Injured Worker was installing an expansion joint into a duct of a preheater. He and a coworker had placed a section of it and then had to slide it into place. The piece weighed approx. 120 lbs. As they were sliding it the IW felt a sharp pain in his lower back.

Employee was using pipe wrench to tighten a fitting. The pipe wrench slipped causing employee's rt. hand pinky finger to be cut on a sharp piece of metal. Employee's injury required five (5) stitches.

The IW was demoing a screw conveyor. The IW had his hands placed on top of the conveyor. Just as the conveyor freed, the IW placed his rt hand on the torch-cut end. The conveyor thus swung towards the torch-cut end smashing the IW's hand between it and some steel supports. The IW received lacerations to the rt hand at the base of the index and middle finger.

Employee was walking on the crane deck to inspect the equipment when he slipped on 4" drop on the platform and fell.

Employee was attempting to close the door on the bottom of the kiln feed shelf of the PreHeat Tower. Employee pinched his Rt. thumb between the door and the door frame causing thumb to bruise under nail. Employee was sent for examination where his Rt. thumb nail was drilled to release pressure.

Employee was picking up a brick off of a stack of bricks while turning with brick in his Rt. hand he struck the forktruck lift bars causing a laceration/contusion on the tip of his thumb. He was taken to hospital and received five (5) stiches in his thumb. He did not miss work.

Employee was bent over grinding a 1/2" steel plate when he felt a sharp pain in his lower back. Strained back after being bent over in an awkward position.

Employee was walking into the tool crib to return a harness and lanyard. When he handed the harness to the tool man, the lanyard struck the employee in the mouth, chipping his front tooth.

Employee was attempting to unplug the PreHeat Tower of hot raw feed material with a water blaster gun on 6/8/06.Ee was wearing his PPE when material flew back falling inside his glove burning ee's lt wrist/top of hand. Ee was sent to hospital where he was diagnosed with 1st & 2nd degree burns. He con't at his regular duty returning for follow up burn area was

Employee was on top of the new silos measuring. He does not know if he bumped his left shoulder or if he was bitten by an insect. The next day he awoke and his shoulder was infected and swollen. He was sent to the hospital ER where he was given a tenanus shot and IV fluid of antibiotics. Employee has remained at full duty.

Employee was carrying a fan to a job site when he stepped into soft mud causing a strain to his right knee. Employee continued on regular duty with restrictions of no prolonged standing or stairs. Continued discomfort returned employee to doctor and surgery was scheduled for 5/4/06, when employee started missing work.

EE bent to lift plywood off the ground and felt sharp pain right side of neck from ear down to his collar bone.

EE was using a pry bar to place shims under the motor of an ID fan, when the pry bar slipped and he fell from a platform approx. 4' to the ground.

EE WAS CARRYING A CUP OF COFFEE IN HIS LEFT HAND AS HE ENTERED HIS CONTROL LOADING BOOTH. HE WAS IN THE PROCESS OF SETTING HIS COFFEE CUP DOWN WHEN HE STRUCK THE CUP ON THE CORNER OF A METAL CONTROL BOX, CAUSING THE CUP TO BREAK IN HIS HAND. EE REQUIRED 4 STITCHES IN HIS LEFT HAND.

Employee reported tingling and numbness in left hand on 11/30/04. Employee has remained at work at full duty while condition was determined. He had surgery on 12/19/05 for carpal tunnel release and is off work.

Employee was walking on snow and ice-covered roadway to the rock storage building when he slipped and fell striking his right knee. Employee continued on regular duty. Discomfort in the knee continued; he was examined and doctor scheduled surgery on 1/18/06 to remove torn cartilage. Employee is off work as of 1/18.

EE strained back on 12/03/05; was able to continue regular duty. 12/20/05 - doctor ordered light duty restrictions. From these restrictions EE cannot perform all the duties connected with job requirements. EE was attempting to lift a frozen piece of material off a belt when he felt strain in lower back. 1/5/06: new restrictions & meds. - EE will have lost work days.

EMPLOYEES WERE STRIPPING CONCRETE FORMS FROM AN ELEVATED PLATFORM. AS INDIVIDUAL WAS PRYING A FORM LOOSE, A 2ND CAME LOOSE AND FELL A COUPLE FEET, STRIKING ANOTHER EMPL0YEE IN THE BACK OF HIS HEAD - BENEATH THE RATCHET OF HIS HARD HAT. EMPLOYEE WAS GIVEN 1ST AID THEN TAKEN TO HOSPITAL FOR OBSERVATION - BLEEDING HAD STOPPED. DR DECIDED TO APPLY 2-3 STAPLES.

EMPLOYEE WAS WORKING WITH ANOTHER EMPLOYEE ON THE CUTTING SHEAR. INJURED WAS MEASURING FROM BACK SIDE OF MACHINE. PIECE OF STEEL WAS SLID IN FROM THE FRONT AND HIS ELBOW WAS STRUCK BY THE PIECE OF STEEL. STITCHES WERE REQUIRED AND HE RETURNED TO REGULAR JOB.

EE WAS CUTTING A HOSE OFF THE VACTOR TRUCK TO CHANGE IT. IN THE PROCESS OF CUTTING THE HOSE, HE CUT HIS FINGER. HE WAS USING A POCKET KNIFE THAT DID NOT STAY LOCKED AND CLOSED ON HIS FINGER, THE LACERATION TO HIS RIGHT FINGER REQUIRED STITCHES.

EE WAS WORKING AT THE AIRSLIDE. STANDING ON TOP OF AIRSLIDE, FOOT SLIPPED & HE STARTED TO FALL, REACHED OUT TO GRAB HANDRAIL. RT ARM WAS CAUGHT ON THE HANDRAIL & PUSHED BACK CAUSING STRAIN / FRACTURE TO THE BONE IN RT SHOULDER. EE HAS RT ARM IN A SLING & IS ON RESTRICTED DUTY. NO RT ARM USE FOR POSSIBLY 10 DAYS - 8 WEEKS OF LIGHT DUTY - TREATING WITH ORTHOPEDIC DR.

EE felt a strain to his neck on 11-2-04 was sent to occupational health for exam. Exam and x-rays did not show any injury. Continued complaints of tignling in his arm he was put in the janitorial position light duty. he became a reportable and lost time on 11-24-04 and has returned to work 12-1-04. still remains in the janitorial position.

Employee was lowering scaffolding lost balance and bent backwards feeling strain. Reported on 10-28-04 this occurred on 10-27-04. Went to his doctor on 11-3-04. An imbilical hernia was the possible injury, ee decided to be treated by his doctor and have surgery on 12-2-04.

EE REPORTED HE WAS WALKING INTO THE ENTRYWAY OF CLINKER TUNNELS AND SLIPPED ON LOOSE CLINKER, STRAINING HIS RT. KNEE. WAS SENT TO HOSPITAL FOR EXAMINATION, BONE FRACTURE WAS DETERMINED AND HE WAS REFERRED TO IV-ORTHO. HE WORKED LIGHT DUTY 3 DAYS, LOST TIME BEGAN 8/27/04. HE RETURNED TO FULL DUTY ON 10/25/04 AND STARTED MISSING AGAIN 12/20/04 DUE TO PAIN. 2ND SURGERY 12/30/04.

EMPLOYEE ENTERED RUTS IN HAUL ROAD CAUSING TRUCK TO ROCK BACK & FORTH. EE'S LT. THUMB WAS CAUGHT IN STEERING WHEEL & FELT STRAIN IN BACK. EE WENT TO CHIROPRACTOR WHO TOOK HIM OFF WORK FROM 7/27/04 TO 8/4/04. HE HAS NOT RETURNED TO REGULAR DUTY DUE TO RESTRICTIONS.

Employee was cleaning up weeds and brush when he came in contact with poison ivy. he went to occupational health at the hospital and given an injection.

EMPLOYEE WAS LIFTING ON BIG TIPPIT ON PREHEAT TOWER WHEN HE FELT A PULL IN HIS LOWER LT. ABDOMEN AREA. WAS EXAMINED AT OCCUPATIONAL HEALTH, SENT TO A SPECIALIST, DETERMINED A HERNIA, SURGERY SCHEDULED FOR 5/20/04.

EMPLOYEE WAS NOT WORKING HIS NORMAL JOB. THE PLANT WAS IN A PLANNED SHUTDOWN WHICH REQUIRES THE ROAD TRUCK DRIVERS TO TRANSFER TO THE PLANT. EE WAS WORKING ON DRAG CHAIN AND LIFTED LARGE SADDLE. EMPLOYEE COMPLAINED OF DISCOMFORT IN NAVEL AREA. DR. DETERMINED IT TO BE A IMBILLICAL HERNIA & FURTHER TREATMENT WAS NOT REQUIRED.

EMPLOYEE WAS EXAMINED AT E.R. & X-RAYS WERE NEG. EE SAID HIS NECK WAS BOTHERING HIM SO HE WENT TO THE CHIROPRACTOR ON 4/7/04. EE WAS OPERATING HAUL TRUCK ON MUDDY GROUND WHEN TRUCK DROVE INTO DEEP RUT & CAUSED EE TO ROCK BACK & FORTH IN CAB & STRIKE LT.SHOULDER IN CAB.

EMPLOYEE WAS CLEANING MUD OUT OF EXCAVATOR TRACKS WITH SPADE SHOVEL WHEN POKING HIT METAL & STRAINED HIS RT. SHOULDER. EE REPORTED THE ACCIDENT HAPPENED ON 2/25/04, HE REPORTED THE INCIDENT ON 3/5/04. EE CONTINUED HIS REG.DUTY W/ RT. ARM RESTRICTIONS UNDER DR.'S CARE. DUE TO SURGERY EE BECAME A LOST TIME AS OF 3/25/04.

EMPLOYEE WAS CLEANING MUD OUT OF EXCAVATOR TRACKS WITH SPADE SHOVEL & WHEN POKING HIT METAL AND STRAINED RIGHT SHOULDER. EE REPORTED THAT ACCIDENT HAPPENED ON 2/25/2004 HE REPORTED INJURY ON 3/5/2004. DUE TO NEW SPPLIED RESTRICTIONS AND UPCOMING SURGERY EE BECAME A LOST TIME AS OF 3/25/2004.

EMPLOYEE HAD FOREIGN MATERIAL IN EYE. HE REPORTED IT TO US ON 1-6-04. TOOK TO EYE CLINIC, EYE WAS RINSED OUT AND EMPLOYEE WAS GIVEN EYE DROPS TO TREAT INFECTION CAUSED BY FOREIGN OBJECT. WE THOUGHT HE HAD DROPS TO SOOTH OR FOR NUMBING PURPOSES ONLY. UPON REVIEW, WE FOUND DROPS WERE PRESCRIBED FOR INFECTION ON 2/5/04.

EMPLOYEE WAS LEAVING WORK AND DOES NOT KNOW WHEN OR HOW SOMETHING WAS IN HIS LT. EYE. HE WENT TO THE E.R. AT HOSPITAL. THEN NEXT DAY WENT TO EYE DOCTOR. NOTHING WAS FOUND - EMPLOYEE WAS GIVEN PRESCRIPTION EYE DROPS.

EE WAS TURNING A SCREW WHEN IT BROKE FREE CAUSING HIS HAND TO HIT DOOR FRAME.

FOREIGN PIECE OF MATERIAL WENT INTO RIGHT EYE WHILE WORKING ON TOP OF SILOS. EE WAS SENT TO EYE DOCTOR MATERIAL WAS REMOVED - IT WAS NOT EMBEDDED, EE WAS ISSUED PRESCRIPTION EYE DROPS.

EMPLOYEE WAS SHOVELING AND CLEANING UP AREA AROUND WEST END OF KILN. A FOREIGN PIECE OF MATERIALOR DUST WENT INTO HIS EYE. EE WAS SENT TO EYE DOCTOR. EYE WAS SCRATCHED AND EMPLOYEE WAS ISSUED PRESCRIPTION EYE DROPS.

EMPLOYEE WAS WORKING ON THE #5 BALL MILL REMOVING HEADLINERS ON FEED END. WHILE PRYING HEADLINER OFF THE FACE OF THE WALL, IT SLID DOWN AND HIT EMPLOYEE'S LEFT FOOT.

EE WAS PUSHING A LENGTH OF RUBBER BELTING THROUGH AN OPENING CUT INTO SIDING (METAL). FINGER BECAME CAUGHT IN BETWEEN METAL SIDING AND BELT LACERATING FINGER ON LT HAND. LACERATION REQUIRED SI X STITCHES.

EE WAS PULLING MOTOR ONTO A BASE INT HE LESCHE MILL. STRAINED RIGHT SHOULDER.

EE WAS LIFTING LIDS OFF SILOS SO THEY COULD BE MEASURED -WHEN HE FELT A SNAP IN HIS BACK. HE IS STILL ON HIS REGULAR JOB WITH A 15 LB. WT. RESTRICTION LIFTING OF HIS RT. ARM AND HIS LOWER BACK

EE WAS OPENING DOOR HATCH ON #413 FAN, THE DOOR OPENED AND DROPPED ONTO HIS ARMS.

EE WAS CLEARING BRUSH & TREES AND CAME IN CONTACT WITH POISON IVY PLANT.

EE WAS REMOVING SCAFFOLD FROM WORK AREA HIS BACK PULLED AND EE COULD NOT STRAIGHTEN UP.

EE COMPALINED OF PAIN IN GROIN DOES NOT REMEMBER A SPECIFIC DATE OR TIME. EE WAS DIAGNOSED WITH A HERNIA ON 7-1-02. EE HAD SURGERY ON 7-5-02. HAS NOT RETURNED SINCE SURGERY.
