WHILE BACKING OUT OF THE FIRST HALFPASS, THE VICTIM EXITED THE BOLTER COMPARTMENT AND WAS THEN PINCHED BETWEEN THE MINER TAIL AND THE RIB. ACCIDENT RESULTED IN DEATH TO THE EMPLOYEE.
