EE WAS DIGGING OUT BLOCKS FROM BLOCK STACK. HE HAD REMOVED TWO BLOCKS FROM THE STACK, AND PARKED HIS OADER ABOUT 15 FEET AWAY FROM THE BLOCKS. HE THEN GOT OUT OF THE LOADER TO EXAMINE THE TWO BLOCKS WHICH WERE ABOUT 4 FEET APART. THE BACK BLOCK WAS UNSTABLE AND TIPPED FORWARD FATALLY CRUSHING HIM BETWEEN IT AND THE FRONT BLOCK.
