THE VICTIM WAS POSITIONING THE WEDGE BAR AND BOLT ASSEMBLY TO SECURE THE UPPER PLATE IN A STATIONARY JAW CRUSHER. APPARENTLY THE LINER SHIFTED AND FELL, STRIKING THE VICTIM.
