A FATALITY OCCURRED AT THE WINDFIELD SOUTH MINE ON AUGUST 21, 2001 AT APPROX. 11:30 A.M. MECHANICS HAD BEEN ASSIGNED TO TURN THE DRAG ROPES AND PUT ON ONE DUMP ROPE ON THE D-22 DRAGLINE. THE D RAGLINE BUCKET WAS CONFIGURED SUCH THAT THE ARCH TO EAT. A CLOD OF DIRT FELL FROM THE BUCKET. APPARENTLY STRIKING THE VICTIM ON THE BASE OF THE NECK. EE ON THE SCENE IMMEDIATELY ATTEMPTED TO
