A METAL PLATE - 15' X 8' X 1" THICK WAS LEANING TOWARD PLANT. WE BEILIEVE VICTIM WAS TWISTING A CABLE ATTACHED TO PLATE TO REMOVE SLACK WHEN THE PLATE FELL ON HIM. IT RESULTED IN VICTIM'S DEATH.
