Employee was moving crusher break plates in storage building. One of the plates started to fall over and EE pushed it back with EE's foot and felt a pop in EE's knee. EE went for treatment on 03/19/2026 and was diagnosed with a right knee injury and primary osteoarthritis.

Mostrando 50 de 229Employee was removing cast liners within rawmill for replacement. Using a sledge hammer to loosen the first liner, employee struck corner of liner and a piece fractured and came back and struck employee in the nose causing laceration and broken nose.
Employee was cleaning the tracks of the bulldozer with a shovel. EE heard a pop and experienced pain in EE's right arm. Employee continued to work unrestricted until EE went to an orthopedic specialist on 12/30/2025. EE was diagnosed with a tear of the right rotator cuff. The extent of the tear is unspecified. The employee is now on restricted duty.
IP was attempting to find a leak in a hydraulic system on EE's repump truck. While moving a line, hydraulic fluid was injected into right hand index finger.
MCC building for primary crusher caught fire and burned down. The MCC was located in an area that did not damage anything else.

The exact date of the roof fall is unknown because the mine was unoccupied on 12/14/2019 and 12/15/2019. MSHA was notified within 15 minutes of the fall being discovered.
The employee was cutting a metal plate using a grinder equipped with a metal cutting wheel. A piece of debris went under EE's face shield & safety glasses & landed in EE's left eye.
