Employee was replacing sprockets on skid steer, realigning the holes when the track shifted and rolled the sprockets over trapping EE's right hand between the track and sprocket

Individual injured while helping change out a bucket elevator belt. As the old and new elevators were being replaced, they got stuck together, causing them to bind. The tension caused them to abruptly come apart, striking individual on right hand thumb area causing a fracture to the thumb. Injured party was transported to get evaluated, x-rays taken confirming fracture to thumb.

While placing a diester insert and attempting to maneuver in place, IP went to grab a tool and as placed right hand along the top railing area to maintain three points of contact, the suspended load shifted as it was shimmied in place, causing load to come down approx. 3-6" on hand, causing lacerations to top of fingers, and comminuted fractures to ring and pinky fingers.

Dryer trunnion guard rails had been taken down in order to replace unit. Maintenance technician was finishing up the job greasing unit and as EE turned and walked away, EE tripped and fell 8-10 feet from a platform, fracturing EE's left wrist and having a contusion over EE's left eye. Taken to area hospital where EE was treated and released same day.

Employee was cutting up some scrap metal when a piece of steel plate had fallen, striking the employee's right foot behind the steel cap on EE's boot. This resulted in a non-displaced fracture of the second metatarsal bone in EE's right foot.

Employee was leaving loadout on a utility vehicle, as EE was reversing, EE was traveling too fast and cut the wheels sharply resulting in the buggy flipping onto the driver's side. Was observed at the local ER for multiple abrasions / bilateral knee strain. Return to full duty with no restrictions.

Employee was checking the 202 conveyor as it was catching on something. While observing it, something popped. This startled the employee and they abruptly jumped back twisting upper body away from it. Employee had immediately experienced a pinching pain in lower back.

Maintenance Tech. was changing troughing rollers on a conveyor, while EE was handing out a bad roller EE's helper had lost grip of the roller resulting in the maintenance tech smashing EE's left hand in between two rollers. Displaced fracture of distal phalanx of left ring finger with a very small evulsion, contusion to EE's left middle finger and pinky finger.
