Employee was exiting CAT 777 rock truck to fill the oil. While climbing down the ladder, the employee's foot slipped off the ladder and employee suffered a rupture of the right proximal bicep tendon.

The injury did not result in any lost time or restricted duty until they went for surgery on 08/23/2023.

He lost his balance when taking the step and fell 11' to the ground. He suffered a laceration above his right eye, a broken elbow and facial fractures.
Employee was exiting the skid loader when he fell, fracturing his wrist. After the accident, the foreman drove the injured employee home and he was then laid off. Allegheny Mineral did not find out about this incident until 1/24/14. Once Allegheny Mineral confirmed that this injury occurred and was not reported, the foreman and injured employee were both terminated on 1/27/14.
Employee was moving a water discharge hose when he tripped and fell to the ground. He struck his face on the ground which caused a cut above his right eye which required sutures.
Employee was climbing into the cab of the excavator. His foot slipped on the track and he fell. He injured his right shoulder during the fall. He was holding the handrail with his right hand. He went to an orthopedic surgeon on 1/28/13 and had an MRI which showed he had a rotator cuff tear. He is on light duty until his scheduled surgery on 2/20/13.
After plant shutdown, the employee inspected area and then began spraying down the plant. A piece of mud was dislodged from the plant and slid into the employee causing him to land on a steel support member. The accident resulted in 2 broken ribs.
The man slipped on ice at bottom of fuel truck and bruised his left knee. He was able to perform his normal duties, but when he was check by doctor, he is on restricted duty which did not interfere with normal work.
The injured man was descending a short set of steps which he slipped on the rain wetted steps and hit his head. There was nothing broken, but he is suffering from amnesia and will be off work until his amnesia clears.
The injured man claims he stumbled on a rock when leaving his car to report to work. There were no witnesses known at this time. He thought it was a sprained ankle. Worked his shift but did not report accident. Went to Emergency Room noon next day & was diagnosed with a broken left fibula. Was cast and he worked normal duty next shift.
The man was walking when he fell on ice. He felt some neck pain but not enough to keep him from working. On 3/21/05 he went to the doctor because the pain was still there. He was given a prescription for pain and relaxation. No restrictions.
THE MAN WAS DISMOUNTING A CAT D6 DOZER, WHEN STEPPING FROM THE TRACKS TO THE BLADE ARM WITH OUT 3 POINTS OF CONTACT, HE FELL AND CRACKED A BONE IN HIS RIGHT WRIST. A CAST WAS APPLIED AND HE IS BACK TO WORK AT SAME JOB WITH NO LOST TIME.
THE INJURED SLIPPED AND FELL, LACERATING A FINGER ON A SHARP ROCK. SURGERY WILL BE REQUIRED TO REPAIR A TENDON. WILL BE OFF WORK POSSIBLY 2 WEEKS.
THE INJURED MAN SLIPPED AND FELL DUE TO SNOW AND HURT LEFT BUTTOCKS. RETURNED TO WORK WITH NO LOST TIME BUT VERY SLIGHT RESTRICTIONS. ACCIDENT WAS NOT REPORTED UNTIL 2-6-02.
