The injured employee was handling the crosshead hook on the #2 bucket for inspections on the bell line. As the employee was walking backward with the crosshead hook, their right boot caught on a "lip" of the collar doors. This caused the employee's knee to hyperextend.

Mostrando los 7The employee caught a boot on the top of a short retaining wall and tripped and fell. The injured struck forehead on the forks of forklift causing a laceration that required three sutures to close.
While working in the shaft the miner fell from height while attempting to access the bench to reattach the bail on the muck bucket. The miner landed on both feet on the muck pile and then rolled. Injuries included a hairline fracture to the left Tibia and a sprained ankle. The miner was released to light duty.

Public Electric utility crew was performing maintenance on electric transmission tower. Tower was located in remote area separate from mine. While descending the hill on which the tower is located, Employee slipped and fell down steeply inclinded surface 45 ft.
